Discharge – during your stay your Treating Team will collaborate with you to plan for your discharge. Should you have any concerns with your discharge plan please discuss with you treatment team. If you require community services post discharge, these will be organised prior to your discharge . Discharge time is 10am.

Towards the end of the pateint‘s stay in hospital, the clinical team will work with the patient to plan their discharge. If any community services are required after discharge, these will be organised as a part of the process.

Where required and possible arrangements will be made for patients to go into supported accomodation post-discharge.

Aftercare Program
All patients are strongly encouraged to attend the Aftercare Program which is held every Monday evening at the hospital. This program assists patients with their ongoing recovery.

Day Program
Where relevant, patients are regerred to various day programs run by the hospital. These programs address areas of specific need of patients, helping them develop and stay in recovery.

   

Discharge at Own Risk – Patients may discharge themselves from the hospital at any time.  This is a serious step when taken against the advice of the doctors and requires careful consideration.  Patients are encouraged to discuss the implications of this with the doctors and nursing staff.  If patients decide to discharge themselves against advice, they will be required to sign a form disclaiming the hospitals responsibility for their action.
