You will be advised by your doctor when you are to be discharged from the hospital. As you leave, please make sure you advise the nursing staff and check with staff at Reception regarding your account.
Don’t forget to take with your discharge envelope which includes:
  • Discharge letter
  • Referral form
  • Medications
  • Community services
  • X-rays, reports and devices
    If appropriate, ensure you have made a follow up appointment with your doctor.

 

    Getting Home
Your transport arrangements will be discussed with you and your family as part of our discharge
planning. It is best to have someone collect you, or take a taxi. Taxis can be booked at Reception.
Ambulances will be arranged by the nursing staff.
